Brief Description of
Duties
The position of Case Manager (PATHS) consists of services that help Temporary Aid to Needy Families (TANF) customers gain access to needed medical, social, educational, and other services. Additionally, the incumbent will be directly involved in providing oversight of an assigned caseload that is directly linked to appropriate services. Case Management includes: (1) assessment of the eligible customer to determine service needs; (2) development of an individualized integrated service plan (ISP); (3) referral and related activities to help the customer obtain needed services; (4) monitoring and follow-up; and (5) evaluation. Case Management does not include direct delivery of medical, clinical or other direct services; it primarily involves customer advocacy.
 
ESSENTIAL DUTIES AND RESPONSIBILITIES
To perform this job successfully, an individual must be able to perform each essential duty at a level of satisfaction that meets the terms and conditions of the Memorandum of Agreement (MOA) between the District of Columbia’s Department of Human Services (DHS)/Economic Security Administration (ESA) and the University of the District of Columbia (UDC)/School of Business and Public Administration (SBPA). Duties and responsibilities include the following:

  • Consistently exercises discretion and judgment to analyze, interpret, and ultimately decides what actions are necessary based on the varying facts and circumstances of each individual case;
  • Works with supervisory assistance from the PATHS Program Manager and the Supervisory Case Manager (PATHS) in evaluating possible courses of conduct and making appropriate decisions that best meet the needs of the customer(s) being served;
  • Maintains an active caseload providing interventions as needed and within area of expertise and limits of credentials; assure procurement of additional services as needed;
  • Acquires and provides to the PATHS team the detailed information regarding an assigned customer to establish the foundation for the Individual Service Plan (ISP);
  • Works with the customer on a day-to-day basis using professional judgment and discretion to implement the team determined Treatment Plan;
  • Assists in development, implementation and revision of individual service plans; assures that services provided are specified in the Treatment Plan and monitors progress toward treatment goals;
  • Consults and cooperates with community systems to facilitate linkage, referral, crisis management, advocacy, and follow up with the focus on attaining ISP goals;
  • Provides crisis management for customers; makes linkages for interventions as appropriate;
  • Maintains and documents customer and program records in accordance with applicable standards and regulations and grant requirements;
  • Maintains a high level of ethical conduct regarding confidentiality, dual-relationships, and professional stature;
  • Participates in continuing education activities, remaining knowledgeable in area(s) of expertise;
  • Attends staff meetings as appropriate and meets regularly with supervisor and other team members to exchange pertinent information and receive supervision as appropriate;
  • Completes required training and ongoing certification as needed;
  • Completes paperwork and reassessments in a timely manner and within specified deadlines;
  • Performs other duties as assigned.


Required Qualifications
 

  • Bachelor's degree in Psychology, Social Work, Sociology, Family/Customer Sciences, Child Development, Substance Abuse, Mental health, Human Services, Education, Criminology, Counseling, and Therapy or related field(s)
  • Knowledge and skills in community-based behavioral health care and case management preferred for Team Leads
  • Good interpersonal and communication skills
  • Flexible scheduling required

Condition of Employment: 
Temporary employment is restricted to a one year appointment and employment may be ended at anytime with or without cause.
