Brief Description of Duties

The Supervisory Case Manager (PATHS) coordinates and oversees evaluation, counseling, and referral services for TANF customers. The incumbent identifies resources and referrals for intervention, prevention, case management, and social service systems. Additionally, the incumbent supports a team of Case Managers whose caseloads consist of TANF customers enrolled in the PATHS Program. The position reports to the Director, Institute for Human Services and Delivery, School of Business and Public Administration.

Essential Duties and Responsibilities
 

  • Develops, implements, and evaluates progress towards goals and objectives of customers.
  • Responds to crisis situations and de-escalates, as necessary.
  • Aids in the delivery of the required program services including, meeting the psychosocial needs of the customers.
  • Reviews Case Managers’ case management forms, including documentation and maintenance of updated required audit information and maintaining confidentiality of records.
  • Coordinates customers’ ability to access other community services and completing the referrals for their usage.
  • Tracks referrals for services in the community and coordinates access to those services.
  • Develops, implements, and evaluates progress toward goals and objectives of each client.
  • Coordinates and arranges for activities related to building of social skills in the community, including linkages with self-help groups, churches, recreational, and other programs.
  • Coordinates with other community agencies and organizations, i.e., Department of Human Services (DHS)/Economic Security Administration (ESA), schools, hospitals, other providers in the community.
  • Coordinates with the Outreach Specialists to provide intensive outreach in an attempt to engage clients who do not follow through with post discharge/deflection appointments.
  • Ensures that the Case Managers provide case management services as defined in the plan of care that coordinate, support, and advocate for customers who need multiple services and require assistance in gaining access to and in using mental health, social, vocational, educational, child welfare and other community services and resources.
  • Reviews weekly documentation of Case Managers in the CATCH system for thoroughness and accuracy. This includes ensuring that documentation is accurate, grammatically correct, and submitted in a timely manner.
  • Prepares monthly reports of case management engagement, activities and progress toward goals and objectives.
  • Prepares other reports as specifically requested by the Program Manager or Principal Investigator and/or Department of DHS/ESA.
  • Meets with the Program Manager to review, plan, and prescribe most effective course of action(s) for the customers.
  • Attends in-service training and/or continuing education as required for the position.

Minimum Job Requirements
 

  •  Master’s Degree in Social Work, Health, Human or Education Services required;
  • Minimum of 60 credits in a Human Service field; or at least two years of field experience in a Human Service environment;
  • Licensed social worker (LICSW) and/or licensed professional counselor (LPC) in the District of Columbia preferred;
  • Knowledge and skills in community-based behavioral health care;
  • Strong interpersonal skills;
  • Effective judgment in interviewing clients and coordinating services with agencies within the community;
  • Must maintain a high level of confidentiality as related to customers;
  • Good writing skills for program planning, reporting, and correspondence;
  • Ability to use independent judgment, perform research, investigations, and inspections, and recommend solutions;
  • Ability to think and react calmly in emergency situations;
  • Knowledge and skills in community based behavioral health care;
  • Possess knowledge of and adherence to ethical standards in clinical practices;
  • Comply with policies and procedures of the Licensing Board;
  • Flexible scheduling required;

Condition of Employment: 
This is a sponsored program appointment. The duration of this appointment is subject to grant fund availability.

Collective Bargaining Unit (Non-Union): This position is not in the collective bargaining unit.
